The Shared Living Program at Momentum connects adults with intellectual and developmental disabilities with providers in a family setting.

This residential service is situated in the home of the provider. The home setting fosters a natural environment in which the individual and the provider can work on individualized goals outlined in the Person Centered Plan. These areas can include personal hygiene, meal preparation, medication administration, community integration, transportation to medical appointments and safety skills. 


Administrative Oversight Provided

Momentum provides administrative oversight to all our Shared Living Homes. These services include the following: 

  • Monthly home visits 
  • Initial trainings and annual re-certifications 
  • After hours and weekend support 
  • Behavioral Consultation Services as needed by trained staff 
  • Attendance at all team meetings and service plan development 
  • Assistance with locating Respite providers 
  • Ensuring prompt invoice submission and payment of bi-weekly stipend 
  • Training and assistance with web-based documentation for daily notes and medication administration 
  • Ensuring the relationship between the SLP and the individual receiving services remains healthy and positive 
  • Coordination with other services being provided to the client 

Individuals Eligible for Services

Individuals eligible for Shared Living services must meet the following criteria: 

  • 18 years of age or older 
  • Diagnosed with an Intellectual or Developmental Disability 
  • Approved funding through DHHS for Section 21 or 29
